Welcome to our Jaw Surgery Before and After photo gallery.
Jaw surgery may be an option if you have underlying jaw position issues that can’t be corrected with orthodontics alone.
Jaw surgery is also known as “orthognathic surgery”. The term “orthognathic” is based on the Greek words ortho, which means to straighten, and gnathic, which refers to the jaw.¹
If you’re considering jaw surgery, Dr Fine will refer you to a qualified oral and maxillofacial (jaw surgeon) for a consultation. Dr Fine and your jaw surgeon will work together closely to develop a treatment plan designed to achieve your goals.

Our patient wore a fixed palate expander, followed by braces and upper jaw surgery to correct a significant underbite.
Before orthodontics and surgery, the patient had a crossbite and ‘reverse’ overbite, with the top teeth all biting inside the lower teeth. After the surgery, both the bite and overall alignment have been optimised.
Interestingly, patients with ‘reverse’ overbites (or underbites) often believe their lower jaw is too big. Certainly, in some cases, the lower jaw is prominent (known as mandibular prognathism). However, after close analysis, in this case, Dr Fine determined that the upper jaw was recessive.
Treating the upper jaw also improves soft tissue support for the nose and cheekbones. These subtle changes can be seen in the facial profile before and after photos above.
As each case is unique, Dr Fine and your jaw surgeon will work together to provide a treatment plan that treats the underlying cause of your orthodontic issues, and provides the most effective aesthetic and functional solution.
Notes: Class III malocclusion. Expander 3 months, Holding Arch (TPA) 7 months, Braces upper and lower 26 months with maxillary surgery (by an oral maxillofacial surgeon).

Orthognathic surgery is generally undertaken during a course of orthodontic treatment. So, patients will usually wear fixed braces, or removable clear aligners.
Surgery usually takes place towards the end of your orthodontic treatment. Your orthodontist will liaise with you and your surgeon regarding the most ideal time to book the surgery. Usually this is once all your teeth are aligned, ready to bite together correctly in your new jaw position.
Once the surgery has been completed, you’re almost there!
You’ll usually need a small number of final orthodontic adjustments to fine-tune your smile and bite.

In some cases, recessive or unbalanced jaws (and associated overbites) can be corrected via combined orthodontic / orthognathic surgery treatment.
Usually, the planned surgical movements can be limited to one jaw; however in some cases there are advantages to considering a two-jaw approach.
